???? The dentist is coming on September 24- September 25.


???? If you want your child to see the dentist please complete the application that they will bring home today.

???? If you do NOT want them to see the dentist please check “no” and sign and return the top page permission slip.

????If you have previously completed an application or permission slip for a previous school year for your child, but do not want them to be seen this year you MUST return the top page and check “no”- otherwise the dentist can use your prior permission slip.
